Job summary

We are seeking a highly motivated, enthusiastic and experienced Band 4 Cardiac Rehabilitation Exercise Facilitator to work within the Cardiac Rehabilitation team. You will be a part of the therapy service and will work alongside Specialist Nurses and Physiotherapists to deliver all aspects of the cardiac rehabilitation programme.

The team is based within the world-renowned Essex Cardiothoracic Centre at Basildon University Hospital and provides a comprehensive rehabilitation programme for patients following cardiac events or surgery. Rehabilitation is delivered in the gym, virtually or via telephone clinics.

The team offers a friendly and supportive environment to enable staff to develop the skills required to deliver high-quality, evidence-based rehabilitation in a variety of delivery modes, according to national guidelines.

Main duties of the job

The Cardiac Rehabilitation Exercise Facilitator will be involved in all aspects of cardiac rehabilitation education and exercise delivery, using a variety of delivery methods depending on service requirements.

  • To undertake assessment of patients prior to attending the gym based cardiac rehabilitation programme under the direction of a registered healthcare professional
  • To lead group exercise sessions (both in person and virtually), while ensuring each patient is exercising at a safe & effective level, which meets their individual needs following British Association for Cardiovascular Prevention and Rehabilitation (BACPR) guidelines
  • To monitor patients throughout the exercise session, adapt exercise and take appropriate action if a patient becomes unwell
  • To deliver home exercise programmes, education and support to patients in their home setting via telephone clinics
  • To deliver education sessions to the patient group during the cardiac rehabilitation programme
  • To participate in the collection of data for surveys or audits, including information for the NACR database
