Job summary
An exciting opportunity has arrived to join the expanding Fracture Liaison Service at Southend Hospital.
You will be responsible for administration of the Fracture Liaison Service (FLS). The purpose of the FLS is to systematically identify and assess all people age 50 yr and above who sustain a fragility (low impact) fracture; and to initiate or recommend treatment, and refer patients on to other services as appropriate, so that the risk of further falls and fractures is reduced as far as possible. The Co-ordinator will identify these patients using various MSEFT IT systems, register them onto the FLS internal patient platform, and communicate with patients, including by booking clinic appointments, sending out clinical questionnaires, and responding to queries from Patients, GPs and other clinicians, either directly themselves or by assigning queries to other members of the FLS team as appropriate.
Main duties of the job
The FLS Co-ordinator will assist the FLS Nurses in downloading and analysing data from the MSE Patient Platform and FLS-DB, to produce regular reports for audit and quality improvement.
The FLS-co-ordinator will type and send letters, texts and other communications to patients, carers and other clinicians as needed.
You will liaise with the FLS CNSs, and with Medical staff, other Nurses and CNSs, and Allied Health Professionals in MSEFT Foundation Trust working with people with osteoporosis and fragility fractures, Primary Care and community care clinicians, and with patients and their families and carers, to work towards excellence in patient care and fracture prevention.
