Job summary

The MSE Integrated Discharge Team (based at Brentwood, Billericay, Thurrock) are looking for a new Facilitator to support with discharge planning.

The role involves working within the Integrated Discharge Team offering support to the Assessors within the team undertaking specific duties involving both direct and indirect care as identified by the Assessors.

Facilitators will also support the delivery of safe and effective discharge for patients from the acute trust.

Applicants must have a valid UK driving license with access to a vehicle as travelling to different sites will be required

Main duties of the job

The Integrated Discharge Team works with patients and families with complex needs and conditions, supporting them in the discharge process in order for a smooth transition in to the community.

The Integrated Discharge Team provides guidance and professional support to the patient and the patient's families, and works with them in partnership to make sure the patient gets the most appropriate plan in place for discharge, whilst ensuring that interventions are not duplicated. This process improves the quality of care as well as improving patient satisfaction.

We aim to anticipate, co-ordinate and join up health and social care needs for patients at a high risk of unplanned admissions to hospital.

We work closely with our system partners and this is recognised in a number of Trusted Assessment schemes supporting the seamless and safe transfer of care of patients between health and care providers.

We are looking for a new member to the team who is patient focused and keen to be involved in a dynamic environment where there is a constant drive for quality improvement to positively impact the outcomes of our patients

You will ensure the delivery of high quality, clinically effective and holistic care, through effective collaboration with multidisciplinary colleagues in every day practice.

In conjunction with the IDT Assessors, providing support and training to healthcare assistants as appropriate to their experience.

To contribute to the basic assessment and development of care plans for patients referred to the Integrated Discharge Team

To provide planned advice, support and instruction to patients, external partners including care homes, care agencies and equipment services and where relevant their carers and other professionals, to promote understanding of the aims of their discharge plans to ensure a consistent approach to care and discharge.

Prioritise own case load and provide facilitation to IDT Assessors when and as needed.

Educate and promote patients and carers to participate effectively in discharge Planning where appropriate.

To gather core information to contribute to discharge plans for patients, including EDDs and MFD dates and feedback to the team. To be competent in completing Assessment and Discharge Notifications in line with the Care Act.

To liaise effectively by establishing and maintaining professional working relationships with other members of the team, patients, carer, and relatives. This may include staff from other agencies or localities when a patients care is being transferred.

Liaise with hospital and community based staff as appropriate to ensure continuity of care and assist in facilitating discharge in a timely manner

To evaluate emergency situations and act accordingly working within procedures and report to the appropriate member of staff on duty.

To promote the well-being of patients and their families by ensuring good communication whilst maintaining confidentiality at all times

Maintain and update patients records showing a clear sequence of events and reasons for decisions taken in accordance with the Trusts guidelines

To be proactive in regards to own development and maintaining professional knowledge base, including having a detailed understanding of Integrated Care protocols.

To ensure personal targets/objectives are achieved and make a positive contribution to multi-disciplinary team, achieving projects goals/targets while supporting delivery of patient outcomes.
