The Integrated Discharge Team are advertising for 1 administrator at our Broomfield site to cover maternity leave from dates 29/04/24 to 27/01/2025
The role is to provide clerical and administrative support and facilitate the smooth running of processes within the Integrated Discharge Team.
The Integrated Discharge Team support patients with complex healthcare needs, who may require long-term health and social care planning or a short term intervention following a change of condition or acute illness.
We aim to anticipate, co-ordinate and join up health and social care needs for patients at a high risk of unplanned admissions to hospital.

Main duties of the job
1. Communication - Maintain effective communication with colleagues and patients.2. Analytical and Planning - To update and disseminate the updated information with the Discharge tracker to all members of the MDT three times a day3. Information Resources - Ensure entries into the computer system are timely and accurate maintaining patient and clinical confidentiality at all times.

- To compile and update the Discharge tracker throughout the day liaising with all multi-disciplinary teams, to ensure all information within the report is accurate.
- Liaise with all wards to ensure Medically Optimised forms are compiled in a timely way by providing advice and support to staff on the correct process following the guidance in the Community Care Delayed Discharge Act.
- To undertake the full range of clerical and administrative duties which include dealing with the public via the telephone to provide support and advice, dealing with staff from multi agencies to ensure information is shared appropriately with the Information Governance policy.
- To have a comprehensive range of typing and keyboard skills including Microsoft Office
- To answer internal telephone enquiries, in a courteous and helpful manner, seeking clarification where appropriate.
- Maintain knowledge and skills by undertaking appropriate training and development to provide a high quality administrative service.
- To maintain confidentiality relating to patients and staff at all times
