Job summary

We invite applicants to apply for the permanent position of Specialty Doctor in the department of Medicine for the Elderly at Southend hospital

We have a large elderly population and seen a rise in emergency admissions to hospital and want to provide the best clinical outcomes for patients but to also work with our primary care partners to prevent hospital admission in the first place.

We have recently reviewed how we deliver elderly care services and in addition to our two dedicated older people's wards we have developed an older people's acute service (OPAS) where patients are seen and treated by a dedicated team of Geriatricians, nurses, social workers, discharge co-ordinators and rehab staff, with the aim of staying less than 72 hours. We also have a Day Assessment Unit where patients are seen and treated on the same day.

Our existing team for elderly services includes experienced geriatricians, discharge co-ordinators, nursing staff, social workers and rehab staff all of which are dedicated to providing good quality patient care. The role of geriatricians and other professionals who specialise in care of older people is crucial in implementing best practice in terms of leadership, involvement, education and training.

Your post will enable you to have some flexibility on their job plan by contributing to the acute DME wards, in reach service and Day Assessment service.

Main duties of the job

You will join the multidisciplinary team working in Geriatric Medicine, including doctors, nurses, rehabilitation staff and social workers. You will be expected to play an active role in MDT discussions over patient care and discharge planning.

Geriatric Medicine includes acute inpatient wards, out patients and Day Assessment unit - you may be required to work in any of these areas.

Clinical responsibility will involve the medical assessment of patients and all aspects of care that this involves and the organisation of investigations and onward referral as appropriate and necessary.

You will be jointly responsible, with the other medical staff, for communicating with GPs and other hospital-based doctors, from A&E, AMU and other specialities. and may be required to physically attend the A&E Department or the Acute Medical Unit in order to assess a patient

You will be expected to support colleagues working in the DAU and may be asked to assess patients attending the Outpatient DVT Service or provide emergency support to patients having transfusion reactions in the infusion room.

Organisational Responsibilities:

You will be responsible for generating discharge summaries or onward letters of referral for patients that are sent home. These should be sufficiently detailed as to ensure safe and effective handover of care from the hospital to Primary Care.
