Job summary
Our Care of the Elderly Ward, Florence Nightingale, has a wonderful opportunity or a Discharge Facilitator to join its supportive and friendly team .
As our Discharge Facilitator you will work under the supervision, direction and guidance of qualified nursing staff and ward manager. Initiating, implementing, assessing and carrying out discharge planning for patients from admission and coordinating this throughout their stay in hospital.
You will require a proactive approach to promoting health and wellbeing, detection and prevention of vulnerability, preventing illness, reacting and responding to existing or potential ill health issues and contributing to reduction in length of stay or reducing unplanned hospital admissions.
Main duties of the job
As our Discharge Facilitator you will liaise with other members of the MDT and the patient's family and carers in order to facilitate efficient and timely discharges and support the nursing staff of the ward to ensure speedy discharges are achieved through a combination of the administrative and clinical elements of the role as described below.
You will be the focal person within the ward environment to ensure that the patient and their families are informed throughout the discharge process to ensure a seamless journey and identify, in a timely manner, any onward service requirements and be pivotal in liaising with families social services care providers and other members of the MDT to co-ordinate assessments prior to discharge.
You will require current Information Technology skills to enable the input of data on electronic tracking systems such as 'medworks' and will display exemplar admiration capability and must have a working knowledge of the trust statutory procedures for discharge planning and the procedures for the transfer of care policy.
The clinical elements of the role will be supplemented with the appropriate competency based training to include removal of cannulas, venepuncture and decontamination of bed areas.
