Job summary
We are looking for an extremely motivated individual with excellent communication, interpersonal and organisational skills who is dedicated to working as part of our friendly team.
You will practice autonomously within the Systemic Anti-Cancer Treatment (SACT) service to provide patient-centred clinical care, encompassing the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the department.
You will demonstrate a high level of expertise within the SACT service, providing advice, education and support to staff, patients, their families and carers and have responsibility for ensuring the provision of high-quality care and leading the on-going development of clinical practice and standards of care within the service, including the further development of nurse led SACT clinics policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues.
Main duties of the job
Work collaboratively with primary care colleagues to ensure that services are streamlined and meet patient expectations and provide a high level of communication.
Liaising with all appropriate members of multi-disciplinary team and ensuring a seamless service between acute and primary care services.
Communicating very sensitive condition related information to patients and relatives, requiring high levels of empathy and reassurance. This includes highly complex information that needs to be conveyed in easily understood language.
Ensuring the provision of appropriate verbal and written patient information, thus empowering the patient to make informed decisions.
You will also participate at care group, trust, national level representative for the service.
