We are looking for caring and enthusiastic team members to join our MSE Bridging Service on a permanent basis.
Successful Community HCA's will work unsupervised in the community, providing direct care to patients in their homes. We are looking for staff to cover all areas of Mid and South Essex: Basildon and Brentwood, Thurrock, Mid Essex, Castle Point and Rochford, and Southend.
The MSE Bridging Service operates from all hospital sites within Mid and South Essex NHS Foundation Trust to assist patients who are fit to go home but are awaiting a package of care. Therefore, successful candidates will have the opportunity to work across the entire Mid and South Essex area as part of the wider team.
The service is part of the MSE Integrated Care Department which is involved with several exciting projects and regularly generates opportunities for development.

Main duties of the job
To provide planned advice, support and instruction to patients and where relevant their carers and other professionals, to promote understanding of the aims of their treatment plan / goals and to ensure a consistent approach to care
To provide domiciliary care for patients where they are unable to, encourage patients to become self-caring. To gather core information to contribute to care plans for patients
To undertake specific clinical activities relevant to the clinical area which demonstrate technical and or support skills that have required specific training, where a high level of patients co-operation is required.
Educate and promote patients and carers to participate effectively in self-management where appropriate.
To undertake manual handling and therapeutic handling techniques with patients both single handed and as part of a team.
To prepare, issue and give instruction in the use of aids and equipment within agreed competencies.
To complete exercises with patients as agreed and set out in care plan by Physiotherapist or Occupational Therapist and ensure patient is performing exercises correctly.
To liaise effectively by establishing and maintaining professional working relationships with other members of the team, patients, carers, and relatives

To evaluate emergency situations and act accordingly working within procedures and report to the appropriate member of staff on duty.
To work co-operatively with the Infection Control Team and others in promoting infection prevention and control best practice
To promote the well being of patients and their families by ensuring good communication whilst maintaining confidentiality at all times
To ensure personal targets/objectives are achieved and make a positive contribution to multidisciplinary team achieving projects goals/targets while supporting delivery of patient outcomes.
Under the supervision of registered nurses, ensure patients are helped to move position to prevent discomfort and / or pressure ulcers.
To perform simple wound care as per patients care plan and delegated by a trained member of the team
To prioritise patient visits based on location and urgency to ensure all patients receive expected visits allocated by Clinical Team Manager.
Complete stock control for equipment and ensure that there are sufficient stores available for patient needs.
