An exciting opportunity has arisen for an experienced, innovative, enthusiastic, forward thinking diabetes nurse to join the team at Basildon hospital to support the management of patients with diabetes in hospital and those attending specialist MDT and nurse led clinics.
We are looking for a highly motivated, innovative and compassionate registered nurse with excellent diabetes nursing and leadership skills who is focused on building upon and continually improving quality diabetes care, safety and patient experience. .
Main duties of the job
The post holder will be an integral member of the multi-disciplinary Diabetes Team providing excellent, specialised evidence based clinical nursing practice in diabetes management for patients in secondary care; in hospital, attending nurse-led and MDT clinics be it virtual or face to face.
Working autonomously within the team the post holder will assess, plan and implement high standards of care to patients with diabetes. They will be an informed resource and act as a role model, providing specialist advice to support team members and other medical staff ensuring the patients conditions are reported accurately, that plans of care and instruction are correctly conveyed to ward nurses, medical staff and that prescribed treatments are administered.
Through effective clinical leadership and with a highly specialised knowledge of diabetes management the post holder will facilitate the maintenance and improvement of the quality of care for inpatients by supporting staff to deliver high quality diabetes management, thus increasing patient safety, reducing length of stay and working towards reducing unnecessary re-admissions to hospital.
The post holder will continue to maintain their own clinical expertise in their field of diabetes and supervise general registered nurses/midwives, allied healthcare professional, patients, carers and relatives across the Trust to ensure they understand the specialty area and aspects of care.

MAIN DUTIES & RESPONSIBILITIES
Use specialised knowledge to assess, examine, diagnose and agree an individual care plan/management plan using a patient centred approach to support, educate and empower patients (and their carers and relatives where appropriate) with uncontrolled diabetes, diabetes related complications and/or complex diabetes management needs including renal disease, vascular disease, neuropathic disease, during pregnancy, the post-natal period and those using pump therapy to manage their diabetes effectively in hospital and at home.

- Demonstrate the capacity to communicate effectively at all levels within the organisation and in external settings.
- Communicate effectively with both internal and external agencies and members of the multi-disciplinary team to ensure maximum benefit to patients.
- Communicate effectively & sensitively complex and complex information, issues and ideas relating to diabetes to patients and carers, where there may be significant barriers to understanding and the need for support and re-assurance, to maximize their potential to understand and manage their chronic condition.
- Capability to identify, assesses, monitor risks associated with service delivery and manage these in line with Trust policy.
- Effectively communicate with colleagues on complex matters and issues relating to diabetes.
- Ensure information given and received is accurate, evidence based and patient centred and a record is maintained in the patient documentation.
- Handles patient/client/staff complaints (including investigation and identification of actions) sensitively and is aware of the need to manage such communications as effectively as possible before complaints cause unnecessary distress to any involved parties.
- Develop educational leaflets in relevant formats for staff and patients relating to service and patient pathway.
