An exciting opportunity has arisen for a highly motivated, innovative and committed Band 6 Complex Discharge Practitioner to join our established multidisciplinary team working within Multi-Agency Integrated Discharge Team.

The successful candidate will need to have excellent organisational and negotiation skills, alongside a strong knowledge of discharge processes for patients with complex health and social care needs. An understanding of the National Framework for Continuing Healthcare and Funded Nursing Care and a working knowledge of safeguarding principles are essential. Excellent communication skills are also required to enable the post holder to work sensitively with patients and families during challenging times. The ability to coordinate the multiple services involved in supporting complex discharges is also key.

Person requirements

You should be a registered health or social care professional (e.g. NMC, HCPC or Social Work England registered) with relevant experience, including involvement in complex discharge planning for patients with complex needs.

Main duties of the job

This is a challenging and rewarding role requiring the post holder to work collaboratively with patients, families, and a wide range of clinical and non-clinical staff, including colleagues from the Local Authority, Continuing Healthcare/Personalised Commissioning, Bradford District Care NHS Foundation Trust, and various multi-agency support teams.

The post holder will work across Bradford Royal Infirmary, St Luke's Hospital, and community hospital sites.

The role is worked over 7 days per week, with participation in a bank holiday rota.

Responsible for the provision of specialist discharge planning support for the most complex group of patients with complex health needs on discharge from the acute and community hospitals.

The role entails working across organisations and links with Social Services and Primary Care Trusts and is key in working with family and carers to support the discharge process.
