To work with the Haematology Malignant Team in delivering a high standard of support to patients with cancer. This will involve being in daily contact with patients suffering from a range of blood cancers and those with suspected cancer. The post holder will be expected to support and signpost patients over the telephone, and progress their care pathway in liaison with other colleagues and departments in the hospital. The post holder will also be expected to be involved with audit and Quality Surveillance reviews.
Main duties of the job
To work as a member of the Haematology Malignant Team providing patient coordination and care, for patients with on the Haematology Malignant Pathway
In conjunction with the Clinical Nurse Specialists (CNS) provide coordination of high quality patient care through ongoing telephonic/ face to face assessment and proactive identification of needs utilising basic clinical knowledge, approved tools and procedures.
Collaborate and communicate with the CNS and wider Health Care Team to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of health care delivery.
Single point of contact for patient, carers and the health care team. Triaging enquires to the appropriate department within the hospital or community setting. Actively participate in the provision of routine information and advice, for patients and carers over the telephone, liaising with the CNS as appropriate
- Document and monitor all aspects of patient care coordination and service delivery, supporting data collection for audit
