This is a development role for a band 5 nurse within the Community Cardiac rehabilitation team. it is hoped progression to the band 6 role will be at the end of the probationary period
The post are an ideal development opportunity will offer the successful candidate a chance of caseload work alongside and under the supervision of the band 6 cardiac rehabilitation community nurse case holders in a community setting
Working one to one with patients referred into the service the successful candidate will benefit from being part of a forward-thinking dynamic team working towards the goals and outcome measures of the commissioning pack for Cardiac Rehabilitation. We are a green certified NACR team
The post holds many development opportunities and offers a chance to develop behavioural change skills, community caseload management skills, run exercise classes and work on the wards It is hoped on achieving all aims and objectives the post holder will transition into a ban 6 specialist nurse role within the service

The service is clinic based with occasional home visits You will also work on the ward providing stage 0- 1 of the commissioning pack pathway.

- Experience in working with vulnerable groups

- Heart failure care experience
