The post offers the successful candidate a chance of caseload management in a community setting .Working one to one with patients referred into the service the successful candidate will benefit from being part of a forward thinking dynamic team working towards the goals and outcome measures of the commissioning pack for Cardiac Rehabilitation.

Job responsibilities

The successful candidate will work the hours over 3 days Monday Friday and manage a caseload of cardiac rehabilitation patients within the community setting. The service is predominantly clinic based but we also do home visits , telephone assessments & inreach to the acute service working on the ward providing stage 0 of the commissioning pack pathway.

  • Two years post registration experience, 1 of which must be in CHD care
  • Demonstrate leadership skills.
  • Evidence of ongoing professional development

Desirable

  • Experience of implementing research-based practice
  • Experience of working within primary care.
  • Experience of Cardio exercise programmes.
  • Experience of previous service development.

Skills

  • Must be willing to undertake further training and education as required.
  • Research awareness.
  • Venepuncture or willingness to work towards.
  • Clinical assessment skills
