Job summary
To deliver a high standard of health care supporting Physiotherapists and cardiac rehabilitation Nurses, together with the wider MDT, to assess, plan and implement a structured evidence based exercise and education programme to service users with acute and chronic cardiac conditions across Bradford.
To manage and organise the flow of patients through phase 3 of the cardiac rehabilitation programme, evaluating and adapting interventions, informing lead physiotherapist of any changes or developments in patient's progress in a timely manner.
To aid rehabilitation and improve functional ability, education, self management and confidence of patients.
To work as an active member of the cardiac rehabilitation team. To contribute to team and service development. To provide support to other team members and students.
Main duties of the job
To deliver a structured evidence based exercise programme as screened and delegated by a qualified therapist in line with agreed local pathways and national targets. To manage and prioritise own clinical workload. To lead cardiac rehabilitation exercise classes for delegated patients.
To formulate appropriate treatment goals for patients.
To monitor and progress patients through the exercise programme in partnership with therapists/ cardiac rehabilitation nurses and in conjunction with patients, carers and colleagues. To report any changes back to the qualified staff in an appropriate and agreed timescale. To act upon adjustments or modifications specified by the relevant therapist/ nurse. In some delegated situations, analyse a choice of options to make a judgement about a plan of treatment.
To assist in the instructions and education of patient and carers, and provide information on local resources and support services according to individual need.
To record patient information in accordance with Trust and team documentation standards
To treat patients individually or within a group, in an appropriate therapeutic environment in partnership with staff.

- Experience of leading/developing relevant programmes of lifestyle intervention for high-risk clinical groups.
Desirable
- Therapy Assistant experience in an NHS Setting/ equivalent experience.
- Experience of working within cardiovascular rehabilitation and knowledge of associated treatments, interventions, and outcomes
- Experience of leading group patient education sessions

- Degree Apprenticeship Healthcare Assistant Practitioner - Level 5 Foundation degree Successful candidates who do not have a foundation degree or equivalent in a relevant area will be required to work towards this.

- GCSE Maths and English or Maths and English - level 2 functional skills
- NVQ Level 3 study or evidence of level 3 study or evidence of extensive clinical experience in a health and social care
