Main duties of the job

Our 47 bedded chronic haemodialysis unit is located at St Luke's Hospital and we are focused on delivering renal replacement therapy to a diverse, multicultural group of patients from the City of Bradford & surrounding towns. We are part of Bradford Teaching Hospitals NHS Foundation Trust and so have the advantage of a large multidisciplinary team to liaise with, whilst maintaining our own individual identity.

We are offering an exciting opportunity for a candidate with a Level 3 Qualification, wishing to commence/ progress and/or extend their experience in healthcare and gain knowledge & skills required to care for patients undergoing Haemodialysis.

This post will offer a Health Care Worker joining this specialist area of practice an excellent learning opportunity. You will join a large, friendly, multidisciplinary team committed to ensuring that best evidence based practice influences the care delivered to the patients. The long term nature of the condition enables the team to build professional relationships with the patient group, without impacting on the Professional Code. The successful candidate will be expected to complete a comprehensive training/ teaching package during their initial 6 months to enable them to carry out the role of a Renal Support Worker.

PRIMARY DUTIES & AREAS OF RESPONSIBILITY

General Healthcare Responsibilities

  • Assist individual patients with all care needs within department under the indirect supervision of a registered nurse or other registered professional.

  • Contribute to the ongoing support of patients and their relatives during their time in the department and during transition to other care environments.

  • Provide assistance and support of patients during long and potentially difficult procedures for e.g. tunnelled central venous catheter insertion/removal which require monitoring the condition/progress of the patient during the procedure.

  • Assist with obtaining, transmitting and storing information related to the delivery of a care service. See section regarding Respect for Patient Confidentiality below.

  • Assist and contribute to the movement and treatment of patients to achieve physical comfort and minimise the adverse effects of pressure according to the prescription of care. Report areas of concern to qualified staff.

  • Provide patients with information about the department and specified procedures as necessary, answering questions / providing additional advice as required.

  • Support the nursing team in care delivery to patients in specialist areas of the department to include commencing & discontinuing haemodialysis via both central venous catheter and arterio-venous fistula/graft

  • Assessment of patients condition through measuring and recording observations, for e.g. Blood pressure, Pulse, Blood glucose, pulse-oximetry,using medical devices as appropriate.

  • Accurately record and interpret observations and using initiative, observational skills and judgement respond to patients changing condition and to implement care, according to departmental protocols/care pathways as appropriate, including instigating emergency procedures, and report/ refer to qualified nursing staff any abnormalities as appropriate.

  • Communicate effectively to ensure appropriated handover of information to all service users, including verbal and clear, concise written information, respecting confidentiality.

  • To assist and enable the patient to achieve all activities of daily living encouraging independence and promoting health, whilst maintaining patients dignity with the department.

  • Enable/assist patients to minimise discomfort and pain according to the prescription of care.

  • Participate in clerical duties as required.

  • To be responsible for assisting the team to promote and maintain a safe and clean environment ensuring that equipment and surrounding areas are maintained to a set standard, including clinical and sharps waste and decontamination of specific equipment following Renal Unit & BTHFT Policies

  • To be actively involved in the departments infection prevention and control programme, to achieve local and national infection control standards and targets.

  • Participating in stock control and putting away stock deliveries.

  • Answering the telephone and recording or relaying messages timely and accurately and any other appropriate administration/clerical duties.

  • Contribute to minimising the effect of disruptive, abusive behaviour and the protection of individuals from abuse, complying at all times for Health & Safety/Risk Management detailed below.

  • Be aware and contribute to the clinical governance agenda.

  • Safe handling and transfer of specimens to pathology.

  • Provide safe transfer of patients by escorting to wards and departments as necessary.

  • Undertake a range of clinical procedures where there is a requirement to have knowledge of the procedure, and due to the nature of the procedure being undertaken, there is a requirement to have a knowledge of how to deal with related non-routine activities, such as dealing with complications, answering questions, or monitoring progress during/following the procedure.

  • Such procedures include but are not limited to :

Commencing haemodialysis/haemodiafiltration via central venous catheters and arterio-venous fistula/graft

Discontinuing haemodialysis/haemodiafiltration via central venous catheter and arterio-venous fistula/graft

Assessing exits sites each session & redressing exit sites at a minimum weekly(as per policy).

Administration of On-Line Fluid in the event of hypotensive episode /cramps on dialysis.

Obtaining of blood samples in accordance with unit policies
