You will preferably have management or Leadership experience for this role and be able to act up to support the Community Nurse Lead. Communication is key and we work closely with our larger multi -disciplinary team colleagues particularly our intermediate care team who we give assistance to provide nursing care. Our other close partners include the following:

  • 5 GP practices.
  • An acute hospital at Torbay.
  • The Community Hospital in Totnes.
  • Therapy teams.
  • Social care colleagues.
  • Care agencies.
  • Local charitable organisations.

Main duties of the job

  • The duties include a wide variety of nursing skills including, holistic nursing assessment, palliative care, catheter care, continence care, enhanced wound care, medication administration and carer support.
  • They should preferably be a registered Practice Supervisor and Assessor within the Care Trust responsible for teaching and mentorship of professional students to graduate, and postgraduate level, on profession-specific skills and knowledge within core clinical areas.
  • The role will entail developing and contributing to professional training programmes to meet the needs of an integrated care team and to ensure records are maintained and updated accurately.

We have a vacancy for a Community Nurse Clinical Lead to work within the beautiful Moor to Sea area in South Devon.

Moor to Sea covers a considerable area which is largely rural. The main towns are Ashburton, Buckfastleigh, Dartmouth and Totnes. This post is full-time 37.5 hrs per week at Band 7 AFC.

We work out of three sites based in the three market towns of Ashburton, Dartmouth and Totnes. Due to the large area covered we embrace innovation and technology to help us achieve the goals of our patients.

Our Moor to Sea team comprises three friendly close knit teams. We work closely together to support each other. This is a beautiful and popular holiday area and the workload can vary depending on the season. We belong to the Torbay and South Devon NHS foundation trust and have close links with the wards and departments at the acute hospital at Torbay hospital and the local community hospital in Totnes.

  • To undertake all aspects of practice as an autonomous practitioner including professional and legal accountability for own work.
  • To carry out holistic needs-led assessments and manage a complex caseload, with responsibility for team or onward referral as required, acting as the care manager for identified individual service users.
