Senior Community Children's Nurse

Job summary

The Children's Community Nursing pathway works across Devon to provide a high quality, responsive and dynamic service for children and young people in the community setting.

We are looking for experienced Children's Nurses to join our Community Children's Nurse team which also includes the Special School Nurse team.

We have a Part Time band 6 Special School-Based Nurse post available. Term time or year round positions will be considered.

You should have proven experience of working with children who have complex medical needs and disabilities, be able to demonstrate the ability to work well both within a team environment and autonomously and ideally have community nursing experience.

You will care for children and young people attending special school who may have a wide variety of health conditions. You will work in partnership with the school staff and other health service providers to provide exceptional levels of nursing care to our patients, to provide care close to home and avoid hospital admission. You will promote inclusion and strive to continually improve care with the involvement of patients and families.

The role will be based at Millwater School near Budleigh Salterton but there will be some requirement to support across other school sites and community.

Main duties of the job

  • To provide specialist nursing support, advice and interventions within the multiagency ethos of Children and Family Health Devon services for children and their families.
  • To assist in promoting the health and well-being of children with complex nursing needs, to enable them to achieve the maximum benefit from their educational opportunities.
  • To provide case management of an identified caseload of children with highly complex and/or long term conditions by utilising and promoting evidence based care.
  • To provide nursing care, support, training, advice and education to children, families and carers in order to meet the children's health needs in the school and community environment.
  • To provide children and young people with the opportunity to express their views in an individually appropriate way, having to have their opinions taken into account to develop the services
