Job summary
Exciting opportunity to join a thriving, developing service as a B6 team leader. This will mean overseeing the shift and managing the staff, referrals and prioritising visits all over the Torbay and South Devon area for our patients in their home residence, preventing hospital admissions and aiding hospital discharges. We have the use of 3 pool cars.
We are a friendly, highly adaptable team who deal with various health and social needs of our community patients, whilst working at night.
Regular supervision sessions, both individual and group, that we can all contribute to, so everyone is heard, and we can all learn. The B6 role includes leading these supervision sessions and supervising several staff members, as well as completing return to work interviews after an absence, making occupational health referrals, supporting other team members etc.
The B6 role includes providingholistic nursing assessment, high quality care, support, advice, and information to patients and carers, with the aim of maintaining patients in their own homes, enabling them to enjoy maximum independence and optimum quality of life.
Ensuring the effective co-ordination and provision of the nursing service in the absence of the Zone Nurse Lead and actively participate in the assessment, prioritisation, planning and discharge of complex patients from secondary to primary care.
Main duties of the job
To communicate and liaise effectively with the integrated health and social care team, voluntary agencies, secondary care, G.P.'s, carers and relatives
Working across the Intermediate care and community nursing teams to provide crisis response nursing interventions, assessment, liaison, referral, co-ordination, delegation of care, or placement to prevent hospital admissions. Aiding hospital discharges to ensure patient safety and providing specialist clinical advice to the multi-disciplinary team.
Supporting the other team members through supervision, 1-1s, occupational health referrals, physio referrals, giving advice in challenging situations, using trained skills to resolve conflict effectively and calmly.
Leading by example, being approachable and self-aware, so you realise how your behaviour effects everyone on shift and therefore the quality of care provided to our patients.
To be in charge of the shift, taking referrals, prioritising and allocating the workload amongst the team, considering where the visits are and who can get to the patient the quickest.
Participating in meetings with Universities, PPG, audits, stock control, staff management, together with the team Lead to ensure the smooth running of our service.
