Job summary

To provide high quality consultant delivered care to patients including clinical leadership and supervision of junior medical staff, ensuring the highest standards of clinical care are provided to all patients attending the hospital.

Main duties of the job

The principal duty of this post is to provide a comprehensive clinical service to patients:

- To at all times, comply with the GMS's guidance on 'Good Medical Practice' as amended or substituted from time to time

- To ensure that patient confidentiality is maintained at all times

- On commencement to ensure completion of the Trust's Corporate Induction

- To undertake duties as defined within the job plan and in agreement with the Clinical Lead

- To be proactive in ensuring that all patients receive rapid assessment, treatment and clinical decision making, whilst ensuring our standard to deliver safe, quality care is sustained

- To be responsible for the Clinical Management of all patients under your care.

- To provide Diabetes and Endocrinology specialist support to other inpatient medical teams

- To participate with General Inpatient Medicine on-call rota

- To contribute to Diabetes and Endocrinology MDTs

- To work flexibly with colleagues to meet the needs of the service and achieve Trust and National targets

- To liaise with clinicians and managers within and outside the department to address any issues arising that may affect the services

- To participate in relevant audit and quality assurance processes

- To communicate with local GPs

About us

Torbay and South Devon NHS Foundation Trust is an integrated organisation providing acute health care services from Torbay Hospital, community health services and adult social care.

The Trust runs Torbay Hospital as well as five community hospitals,.

We have around 500,000 face-to-face contacts with patients in their homes and communities each year and see over 78,000 people in our A&E department annually. We serve a resident population of approximately 286,000 people, plus about 100,000 visitors at any one time during the summer holiday season.

Communication and working relationships

Develop strong relationships with patients on caseload to ensure they understand the role and how to make the best use of the relationship

- Develop excellent working relationships with the Diabetes and Endocrinology team but also the GP practices (all staff), team members of the Inpatient Wards, Specialities in the Long Term Conditions cohort and across the hospital, pharmacists, Devon CCG, voluntary and statutory agencies and team members

Planning and organisation

Ensure that patient records are well maintained

- Record action plans for all patients

- Ensure all contact with patients is recorded appropriately on Trust systems

- Ensure that there is a strong system to ensure patients receive planned support and actions are followed up

- Be able to work flexibly and manage own time and workload

Analytical and judgement

- Ensure any concerns regarding a patient are escalated to the most appropriate manager and/or Clinical team/Clinician

- Using professional judgement, ensure patients concerns are managed through to resolution by utilising existing patient pathways and potentially seeking to create new/enhanced pathways

- Work within the parameters of professional registration

- Able to own problems and get involved in the solutions
