Job summary
We are looking to recruit an enthusiastic, motivated, dynamic nurse who can demonstrate commitment to the provision of high-quality person-centred care. You must be adaptable to change, demonstrating good organisational, managerial and leadership skills within the acute setting of elderly care medicine.
Your role be part of an innovative outreach service that enables patients to be discharged home sooner than planned or to avoid a hospital admission altogether. Providinga robust and effective treatment regime for patients requiring I.V antibiotic therapy which encompasses the patient's own individual needs, involving the MDT to ensure cohesive team working results in a successful patient outcome.
Also to run a nurse led clinic for the assessment, diagnosis and treatment of ambulatory DVT/PE patients and to support the cardiology virtual ward in the treatment of heart failure patients.
Full and part time positions available and job share possibility.
Main duties of the job
- To be responsible for the provision of I.V antibiotic therapy for all patients within the MAAT service either in a community or clinical setting
- To be autonomously responsible for the assessment, diagnosis and treatment of patients who attend the DVT and ambulatory PE clinic
- To facilitate and support the provision of I.V diuretics for heart failure patients referred by the cardiology virtual ward in a clinical setting.
- To facilitate integrated working between secondary and primary care, linking up with G. P's, MRU, E.D & the D.G.H Wards
- To autonomously be responsible for the admission and discharge of patients cared for in an ambulatory setting of either their homes or hospital-based clinics
- To initiate, implement and care for patients in the community who would have otherwise been in hospital
- To provide clinical management and autonomy within a culture of integrated nursing and flexibility for this seven-day service
- Be confident in driving own vehicle/Trust vehicle for community visits
- To continuously act as the patients, advocate in the interest of their health and medical well-being. Must clearly document the nursing care given, evaluating its outcome in terms of achievement of the objectives
- Be able to effectively communicate that knowledge to the multi-disciplinary team and to the patient and their relatives

- The post holder will be part of a multi-disciplinary team, working with and supporting all members of that team
- As a sister the post-holder will be expected to ensure that all staff working within the department on a daily basis are able to provide and be provided with mentoring, supervision and teaching of new and junior members of the nursing team
- Responsibility will be to the Senior Sister
- The post-holder will have no direct budgetary responsibility but must be aware of the need to use resources effectively
Key result areas
- These will be identified and agreed at interview and yearly appraisal.
- They will incorporate general management roles that ensure the safe and daily running of the department, for example, managing short notice staff absence.
- These additional responsibilities may change on an annual basis and work towards increasing the management skills of the individual.
The MAAT team is a supportive and caring team of nurses and we provide a service which is much appreciated by patients and their families. Our current involvement in virtual wards also provides us with an opportunity to expand allowing more patients to be cared for at home but to still receive acute hospital care.
