Job summary
This post is to work as a member of the Lipid multidisciplinary team by providing specialist knowledge, skills and advice to improve the standards of care for patients with hyperlipidaemia, as defined by national competencies and guidelines
You will assess, treat and support patients with hyperlipidaemia and agree a care plan/management strategy which will support patients, carers and healthcare teams to manage their conditions more effectively and liaise with and give advice to other members of the team and other healthcare professionals regarding the management of patients with lipid related problems, have knowledge of disease management pathways within secondary and primary care and refer on where appropriate
To support the multidisciplinary team and actively participate in the continued development of the lipid service, contribute towards the clinical supervision, training and education of patients, carers and healthcare professionals within primary and secondary care settings andbe part of a team implementing a family cascade, audit and diagnostic service for the identification and management of individuals with familial hypercholesterolaemia (FH)
Main duties of the job
- To provide FH services, including clinics, counselling and organising biochemical and genetic tests, and provision of treatment under the guidance of the consultant team
- To deliver an evidence-based, NICE-approved specialist service to support primary care in the optimisation of target-driven lipid management for high risk CVD patients, which includes optimisation of statin therapy and the use of injectable PCSK9 inhibitor therapies
- To work with a wide range of healthcare professionals in primary care, cardiology, stroke medicine and vascular teams to optimise care of patients with hyperlipidaemia
- Support patients to achieve shared goals or outcomes
- Support FH patients and their families, providing expert advice and guidance on the management of this condition
- Showing compassion when dealing with distressing or emotional circumstances
