Job summary
We are looking for an enthusiastic and motivated Pharmacist to work in a job share, as the Lead Pharmacist for Frailty at the Bristol Royal Infirmary. The post-holder will co-lead the Pharmacy Service provided to the frailty team, working alongside other members of the MDT and pharmacy teams. Main duties of the job
Your day to day duties will include (but not be limited to):
- To be responsible for providing pharmaceutical care and medicines optimisation to individual patients in the Frailty Service at the BRI
- To work within the Frailty team to reduce admissions, expediate discharges and work across the ICS to standardise care delivery pathways.
- Clinically reviewing patients as part of a pharmacist led consultation using independent prescribing skills to optimise patient care andmaintaining prescribing competencies and prescribe within the Trust's Non-Medical Prescribing Policy
- This post holder will be expected to cover 15 hours per week ideally on Thursday and Fridays to work alongside the post's job share. The exact daily hours worked will be determined according to service need but is likely to be between 8am and 6pm.
- As a member of the Pharmacy team you will also work in our BRI dispensary (as overtime), on a rota basis, during Mon-Fri 5-6pm
- Our Saturday, Sunday & Bank Holiday shifts are part of normal contracted hours with commensurate 'off-duty days', on a rota basis of roughly one weekend day every 6 weeks.

Essential
- Master's degree (or equivalent) in pharmacy.
- Registration with General Pharmaceutical Council (GPhC).
- Evidence of continuing professional development (CPD).
- Postgraduate Diploma or MSc in Clinical Pharmacy
Desirable
- Membership of the Royal Pharmaceutical Society.
- Qualification in Independent Prescribing - NB: To be undertaken on appointment if not already a registered non-medical prescriber
- Physical assessment skills qualification
Knowledge and Experience

- A thorough knowledge of UK hospital pharmacy practice, safe management at transfers of care and medication commissioning
- Clinical experience in managing frail or elderly patients with knowledge of common conditions encountered and of safe and appropriate drug treatments
- Experience of polypharmacy review and appropriate de-prescribing
- Previous experience of audit & quality improvement
- Experience and ability to initiate, lead, manage and supervise a range of projects
- Experience of managing and motivating staff, appraising and giving feedback
- Knowledge of basic medicines information and critical appraisal techniques to apply medical / pharmacotherapeutic evidence to patient care
- Knowledge of the legal framework for the dispensing, prescribing and administration of medicines
- Knowledge of budgetary management and financial control
