We are excited to offer this new opportunity for an experienced nurse or AHP to join the team in Weston. This post is a key role within the wider multidisciplinary team, comprising of ED Consultant, Senior Nurse, and colleagues from specialist areas including the Homeless support team, Drug and alcohol services, Safeguarding, and Liaison Psychiatry. High Impact Users can be frequent attenders to the Emergency Department, those whose presentations are difficult to manage or those with complex medical, psychological or social needs. The post holder will be based within the Emergency Department.
The post was created to provide co-ordination and management of High Impact Users to the Adult Emergency Department, ensuring that this group of patients are identified, safely and effectively managed, and given the best and most appropriate care.
The post holder will use their expert knowledge, skills and initiative to ensure that existing systems and protocols are used for the early identification and coordination of care for High Impact users, and that these systems are maintained and updated as required.
They will participate in meeting the health and clinical advocacy needs of this group of patients , using their high quality Nursing knowledge and experience, ensuring that patients receive the best possible ongoing care, and that ED staff are supported in meeting sometimes complex care needs.
Main duties of the job
- Provide clinical advice based on expert knowledge and skills and support locally in the ED for nursing and medical staff in the management of High Impact users.
- Coordinate the gathering of information from multi-disciplinary team members to assist in and writing Patient support plans- pulling on specialist advice from the required health professionals.
- Liaise with external agencies and Trusts in gathering information about High impact users to coordinate accurate Patient support plans
- Arrange follow up multi-disciplinary team meetings to discuss individuals, this will include meetings across other Trusts to discuss high impact users and share and develop information and patient support plans.
- Monitor previous High impact users' attendances to monitor increases in activity, to review the impact the support plans and work has made on individuals attendance. Review any changes to the service and its impact.
- Participate with the review within division of Weston to evaluate the role with regard to cost-effectiveness of role in reducing readmission rates and in-patient length of stay.
- Write up annual report- include numbers seen, reductions in attendances
