As the Specialty Team Lead Pharmacist for care of the elderly and frailty, and a therapeutic expert, you will provide a dynamic link between clinical practice and service development by creating new models for delivering patient care ensuring the best experience and outcomes for patients.

Within this role, you will lead and be responsible for the development of a team of specialist pharmacists while collaborating with system partners to improve the delivery of care across the elderly bed base and front door services.

You will be required to work additional shifts including evenings and weekends on a rotational basis.

We are seeking Consultant post approval for this position.

Main duties of the job

  • To operationally & strategically lead the provision and development of pharmacy services to care of the elderly care and frailty specialties
  • Provide a clinical pharmacy service to care of the elderly and frailty services by optimising patient outcomes; working within patient care pathways across sectors and healthcare boundaries
  • Contribute to improving health outcomes for patients using a holistic approach to care
  • Ensuring the safe and cost-effective use of medicine through provision of medicines-related aspects of patient care
  • Manage the delivery of pharmaceutical care & pharmacy services to align with the vision and plan of Healthy Weston 2 at Weston General Hospital

Qualifications and Training

Essential

  • Masters degree (or equivalent) in pharmacy
  • Post graduate diploma or Master's degree in clinical pharmacy or equivalent experience
  • Evidence of continuing professional development
  • Registration with General Pharmaceutical Council (GPhC)
  • Accredited at consultant level or to be completed within 18 months of appointment
  • Evidence of leadership development
  • Membership of relevant professional groups e.g. UKCPA

Desirable

  • Independent prescribing qualification, or currently working towards attaining

Knowledge and Experience

Essential

  • Appropriate advanced and current clinical experience in the Care of the Elderly specialty
  • Proven experience of working at a strategic level with senior clinicians and managers and experience of working in an integrated directorate team
  • Proven experience of working at a strategic level across the wider health community
  • Recognition as an expert nationally through publications / presentations or officer in a national group
  • Formal teaching / training/ mentoring training
  • Understanding of NHS finances

Desirable

  • Experience as a clinical tutor for Higher Education Institution diploma

Skills and Abilities

  • Demonstrate advanced level of clinical reasoning and judgement
  • Demonstrate an intuitive grasp of complex situations based on deep tacit understanding
