Job summary

Are you a pharmacist or pharmacy technician looking for a clinical role working closely with patients? Would you like to be integral to developing our anticoagulation service at a time of change? Do you enjoy working in an expert clinical team?

The UHBW anticoagulation service is run by pharmacy professionals with the purpose of optimising anticoagulant therapy for patients across Bristol and we are looking for a pharmacist or pharmacy technician to compliment our team.

This is an excellent opportunity for you to gain in-depth experience dealing with a wide variety of patients and health care professionals in order to safely manage patients with their anticoagulant medications.

Main duties of the job

We are looking for someone with excellent communication, organisational and problem solving skills. Experience with anticoagulant dosing is desirable but not essential as full training will be provided.

Anticoagulation services:

  • To determine the safe and appropriate dose of anticoagulants for referred patients in order to maintain an optimal level of anticoagulation for each individual patient and to minimise adverse effects.
  • Take into account concurrent drug therapy, diet and alcohol consumption, lifestyle, pregnancy status and social conditions in the management of anticoagulants.
  • To refer unstable or uncontrolled oral anticoagulant patients to other healthcare professionals as appropriate in order to maintain an optimal therapy.
  • To accept referrals to the anticoagulant service, create and maintain the relevant written and computerised patient records to ensure accurate & timely documentation of patient records and promote continuity of care.
  • To counsel patients, providing specialist advice, instructions and reassurance regarding the safe and appropriate use of oral anticoagulation in order to achieve concordance.
  • To provide advice on warfarin, DOAC, and heparin dosing switching between various anticoagulants to ward doctors/ non-medical prescribers/ GPs.
