Stroke Specialist Nurse - Nurse Early Supported Discharge Team for Stroke

The Early Supported Discharge Team is an integrated team assessing patients, acting as case coordinators and providing specialist stroke rehabilitation.

This role - to provide specialist knowledge and advice including health promotion, secondary prevention & medication management within a friendly team.Working as the stroke specialistnursedelivering evidence-based person centered care to adults who have had a stroke. The role involves seeing patients within their own homes and undertaking skilled and specialist assessments.The team also works closely with in-patient teams.

This role will promote an integrated approach to stroke care across Bristol.

You will have your own caseload of patients and work with the ESD team setting therapeutic goals and planning programs of care.

As part of the ESD Team this post will transfer across to a community provider as part of the BNSSG stroke programme development of stroke services.

You will need to be a car driver with access to a car for work.

Main duties of the job

To work as a stroke specialist nurse within the Early Supported Discharge Team (ESD) for Stroke, facilitating early discharge home from the acute stroke unit and other UHBW beds. To provide evidence based, person centred care and support for adults who have had a stroke, primarily in their own homes.

To be an experienced clinical nurse resource in the care and management of people with stroke, being able to work autonomously, managing own clinical caseload and providing a link between acute and community services.

To meet with the other clinical nurse specialists within the Trust for clinical support and update.

  • To work as part of the inter-disciplinary ESD Team, working with in-patient teams to facilitate the early discharge from hospital for people following stroke.
  • To be responsible for assessment of new or potential patients. To co-ordinate allocated cases being prepared for discharge home with the team.
  • To manage own caseload of patients who have had a stroke and require direct clinical involvement, support and intervention. This will be in people's own homes and at set times on the in-patient wards.
  • To work with the ESD team in setting therapeutic goals and planning programmes of care, delivering stroke specialist rehabilitation to people in their own homes.
