in the Milford Integrated Care (MIC) Hub, at Milford Hospital

Working
as part of the MIC Hub team you will have the opportunity to develop your
career in a service providing comprehensive Geriatric assessment and IV therapy
to patients in the Guildford and Waverley area working with a team of dedicated
doctors, nurses, therapists and the wider community team.
The
services is expanding in line with the new frailty strategy so you would be
able to help build and develop services while enhancing your own skills and
experience.
We
are looking for a motivated registered nurse with a passion for improving
patient care and patient experience to join our team based at Milford Hospital
in the MIC Hub to provide the highest quality care & assessment.
Main duties of the job
The MIC Hub provides holistic
assessment for older people with a variety of complex needs with the aim to
improve the patients quality of life, promote self-care, reduce risk of
deterioration in patients condition and avoid unnecessary hospital admission. The
team is multidisciplinary so you will work alongside dedicated professionals
including Doctors, Community Matron, Health Care Assistants, therapists, and
administration staff to deliver completed comprehensive geriatric assessments
for patients with frailty.
The MIC Hub also delivers IV drug
administration in the Outpatient Parenteral Antimicrobial Therapy (OPAT) service
and PICC line care, and you will manage these patients in the community as part
of your role.
Full support with transition from
different clinical areas will be put in place as apart of induction including
competencies of clinical skills required. The service runs from 8.45am- 4.45pm daily, including weekends and bank holidays for some aspects of the service.

As a registered, accountable and autonomous nurse to act in the best interests of your
patients at all times and take personal responsibility for your actions and decisions. To
be responsible for patient assessment, planning of care, implementing and supervising
safe and compassionate care, and evaluating the effectiveness of care. You will also
be required to up skill and support more junior members of staff. As nurse in charge
you will have overall responsibility for day-to-day management of your clinical area and
for ensuring all care is delivered according to Trust policies and procedures,
maintaining associated records. You will work alongside and support and at times lead
a team of nurses in the delivery of evidence based patient centred care, which is of a
high standard. You will assist the senior sister as well as other clinicians in the
management of staff including appraisal, rotas, recruitment and selection and the
supervision of learners. As an NMC you will registrant facilitate learning and
development of others. You will demonstrate leadership qualities and be open,

To plan and implement a program of care to optimise Health & Wellbeing and avoid
hospital admission for our frail elderly population. Being part of a Multi-Disciplinary
Team (MDT) approach to referrals for patients who have frailty identified and require
proactive interventions to maintain independence. This is conducted through a
comprehensive geriatric assessment, in an outpatient setting and additional
investigations. To work as a senior member of the multi-disciplinary team, promoting
health, independence and maximizing the quality of life for patients and carers. To
complete an initial triage/consultation for patients referred to the service and
document information, referring on to appropriate services as needed within
community services. Performing assessments as part of the MDT for patients
attending clinics and being an active member of the MDT meetings.
