Job summary

Hospital At Home Band 7 Clinical Lead Nurse

The brand new Hospital at Home service was set up in December 2022 to provide hospital level care at home for our frail patients. Because of this we have a number of Band 7 nursing posts available for dedicated skilled practitioners.

The service gives suitable patients the opportunity to receive acute care and treatment where they feel most comfortable in their own home. Patients will be seen by a variety of skilled healthcare professionals including doctors, nurses and AHPs.

Main duties of the job

We are looking for nurses with Advanced Clinical Skills who are willing to develop further as part of the Hospital at Home Team. You will be expected to assess, plan care and monitor patients in conjunction with their medical plan.

You will be supported and have the opportunity to develop your career further while in post. This is a really progressive new service and we are seeking enthusiastic senior nurses to support its development.

The Hospital at Home team works with the acute frailty team to provide a service in patients However, the majority of referrals are for our frail elderly population. This is conducted through a comprehensive assessment in the patients own and the provision of short intensive intervention. This will be working with the Urgent Community Response team and other services including GPs, Community Matrons and nurses, therapists and rehabilitation assistants Guidance and expanding the team.

The service operates 8am to 8pm 7 days a week and the post holder is required to work shifts within this times.

You will be expected to visit patients that require holistic assessment and care in order to keep the patient at home. These may be patients already known to Adult Community Services who have had a change in their care/nursing needs or new patients referred in from external services.

  • Experience in leading an MDT team.
  • Management of complex patients with a frailty markers and or multiple co-morbidities.
  • Advanced clinical skills with experience of history taking and physical assessment.
  • Experience of working autonomously.
  • Ability to make decisions autonomously, when required, on difficult issues.
  • Experience of working with people with long term conditions.
  • Awareness and understanding of local and national priorities in relation to Nursing.
  • Understanding of health care policy and how this impacts on community service.
  • Awareness and understanding of local and national priorities in relation to integration.
  • Awareness and understanding of local and national priorities is relation to Hospital at Home.
  • Ability to direct and co-ordinate programmes of care working collaboratively and autonomously.
  • Experience of staff management and appraisal.
  • Demonstrable evidence of service improvement/implementing local change Demonstrable evidence of service improvement / implementing local change.
  • Ability to prioritise and managed daily department workload, assess patients care needs and teams workload without direct supervision.
  • Business planning skills.
  • Working as part of a fully integrated MDT.

Qualifications

  • Educated to Degree level or equivalent, specialist knowledge acquired through relevant experience.
  • Current NMC registration.
  • Current 1st Level Registration (Registered Nurse).
  • Evidence of Continuing Professional Development and significant post-graduate experience in community healthcare.
  • Teaching, learning and assessing NMC Assessor/Supervisor.
  • Health assessment level 7.

Desirable

  • Leadership Qualification.

  • V300 Non-Medical Prescribing.
