Job summary
We are looking
for a Specialty Doctor in Geriatric Medicine to join our Ageing and
Health Department. There are currently 8 substantive Consultants (many less
than full time), 3 Locum Consultants and 3 Associate Specialists
providing senior leadership to frailty services across the organisation. Services include the Acute Frailty team,
Frailty wards, Orthogeriatrics, Stroke, inpatient Older Peoples Advice and
Liaison service and Proactive care based at Milford Hospital.
We welcome applicants with at least 4 years experience in general and Geriatric Medicine. We have developed
local Tier 3 training to support career progression for Specialty Doctors and
other allied healthcare professionals in Geriatric Medicine and would envisage
post-holders to may wish to remain in long-term Specialty Doctor posts, or
rotate four monthly through different frailty services developing a portfolio
of competences and progress to become an Associate Specialist or a Consultant
through the CESR route applying for entry on the Specialist Register.
This
is a post at Specialty Doctor level as per the SAS doctors
contract. It is likely to rotate every
four months within Ageing and Health to ensure a wide range of experience. It currently has no weekend commitments but
is likely to contribute to seven day working in the future with day-time
weekend duties (maximum 1 in 4).
Main duties of the job
Supporting
the Consultants in leading and supervising the team in the day to day
management of patients following a comprehensive geriatric assessment approach
Team
members should attend a daily huddle or SAFER board round to ensure
prioritization and efficient use of time
Working
closely with the wider multidisciplinary team to ensure patients are managed
effectively and efficiently proactively
liaising with patients families / next of kin (with patients permission where
they have capacity or otherwise in their best interests) to ensure an
understanding of frailty and management plan
Supporting
junior doctors in their training and completing assessments
All
members of the team are responsible for ensuring patient flow and effective bed
usage by prioritizing discharge planning / completion of Electronic discharge
letters
Working
with iOPAL consultant to provide initial frailty review for patients identified
with frailty on non-frailty wards
