It is an exciting time to
join this team, as we work to implement our Ageing Well and Frailty Strategy across
the Guilford and Waverley healthcare system. Our Acute Frailty Service is among
the best in the country and the strategy has supported us in developing a
Frailty Same Day Emergency Care facility, direct conveyance to this unit by working
in partnership with local Ambulance services, a Geriatrician of the Day model
providing acute clinical triage and a Hospital at Home service. Opportunities
exist to rotate through all of these pathways and prospective cover is within
budget. The MICH hub was developed during the pandemic and looks to support
neighbourhood teams with standardised CGA assessment. We are looking to work
with system partners to risk stratify older people living with frailty, to
proactively offer CGA to them and to integrate with locality based MDTs.
All
new posts will contribute to our Frailty Academy which has been running since
2021 and has embedded frailty training across the organisation and local
system, including the development of the a trainee ACP programme and an
internal CESR programme. We recently won a bid to expand the academy and to
help develop a strategic workforce plan fit to meet the needs of an ageing
population.
Main duties of the job
Post 1: Clinical Director for the integrated crisis pathway.We are looking for a Clinical Lead to support delivery of an integrated acute frailty service including Urgent Community Response, Hospital at Home, our dedicated Frailty Same Day Emergency Care facility and the award winning Acute Frailty Team based in the Emergency floor.
Post 2: Clinical Director for integrated proactive care: We are looking for a Clinical Lead to support development of our CGA centre based in our Milford Integrated Care Hub (MICH). This role will provide key medical leadership to the hub and will also be system facing, helping to shape the Guildford and Waverley proactive care strategy and development of neighbourhood MDTs using population health management.
Post 3: Interface Geriatrician working across the integrated crisis pathway:This is a substantive post based in the integrated crisis pathway, offering rotation through our services including Hospital at Home, Same Day Emergency Care and the Acute Frailty Team.
Post 4: Clinical Lead for the iOPAL service:this post will work in an established Acute Frailty Unit and lead the inpatient Older Peoples Advice and Liaison Service, offering standardised CGA to older people with frailty in outlying wards.
