Job summary

Would you like to join our Case Management and Discharge Hub Team?Do you have previous experience working in a healthcare setting and have an understanding of discharge or do you have experience in administration?

We are looking for a team member to join us to support the discharge and flow of patients within the Royal Surrey NHS FT and community hospital sites.

The Case Management Service works alongside the Multi-disciplinary Team to support the patient's pathway through the hospital. We aim to ensure patients are transferred home or to the most appropriate care setting in a timely manner whilst providing a safe, effective and positive patient experience.

The discharge hub is essential in supporting the onward referrals and day to day communication and administration required to facilitate effective flow from admission to discharge.

As a Case Manager Assistant you will support the Case Managers and the ward staff with completion of referral forms and discharge documents, booking transport, and resolving internal delays.

Main duties of the job

The focus of the Case Management Service is to coordinate seamless patient flow through and from the hospital. The Case Management Service coordinates all patient's pathway through the hospital ensuring patients are transferred to the most appropriate care setting in a timely manner whilst providing a safe, effective and positive patient experience. The principle responsibilities of this team are:

- To improve the patients experience and quality of care through reducing unnecessary bed days.- To facilitate the timely and appropriate transfer of care to the most appropriate care setting- To provide clinical expertise and decision making.

- To provide constructive challenge to internal and external teams to ensure patients do not endure unnecessary delays to their transfer of care- To identify, resolve or escalate clinical, operational or external issues that prevent a timely transfer of care

- To provide clinical and professional support and leadership to the clinical areas as appropriate

  • Minimum of NVQ level III in care/BTEC Health and Social care or equivalent
  • IT Qualification e.g. European Computer Driving License
