Job summary
The Patient Pathway Tracker will be part of a team responsible for tracking patient pathways to ensure that cancer waiting times standards are met. The team will also manage the Consultant Patient Upgrade service. The role will involve sourcing and inputting clinical data in order to meet the requirements of the National Cancer Outcomes and Services Dataset and National cancer audits.
Main duties of the job
To actively track individual patients on to ensure National CWT standards and targets are met, and policies are adhered to.
Responsible for ensuring accurate collection of patient data for the national cancer outcomes and services dataset (COSD) for the pathway of suspected/confirmed cancer patients.
Use and recommend development of information and administration systems for the recording and reporting of clinical and performance data.
