Job summary

The Transitional care Support Worker will primarily work alongside the Midwives, Neonatal Nurse Practitioner and mothers in caring for neonates requiring support in a Transitional Care setting.

Main duties of the job

  • The Transitional Care Support Worker will assist the midwife to maintain a high standard of care
  • The Transitional Care Support Worker will be accountable to the midwife that she is working with
  • The Transitional Care Support Worker will maintain the dignity of patients and relatives at all times, respecting their individual needs and wishes
  • The Transitional Care Support Worker will observe changes in the condition of mothers and babies, take the prescribed actions and report promptly and accurately to the midwife in charge
  • The Transitional Care Support Worker will be aware of the Trust guidance and policies on all aspects of Health and Safety including Infection control and COSHH (care of substances hazardous to health)
  • Planning care in partnership with women and registered practitioners, training and teaching, appraisals, providing advice to support discharge of women and babies from maternity care settings, providing specialist support to women and families with complex needs
  • The role holder is able to delegate tasks appropriately to others. They are able to evaluate their own and other's practice and suggest and implement changes to improve service delivery.

An opportunity has arisen to join our team caring for babies and their families within the Transitional Care area of our Post Natal Ward. This is a permanent position.

You will need to demonstrate evidence of a previous caring role with children or babies in a hospital setting. Previous experience in Maternity or a nursery nurse qualification would be an advantage, but training will be given to the successful candidate.

If you are passionate, flexible, have a good sense of humour, have the ability to work well as part of a multidisciplinary team, and can commit to providing a high standard of care we would be delighted to hear from you.

Hours: 36 hours per week (0.96WTE) to include internal rotation to night duty, weekends and holidays

Salary: Band 4
