Job summary

This a fantastic opportunity for an enthusiastic and ambitious person to work in the Early Intervention Team in Psychosis Derby City / County who enjoys working as part of a team and embraces and supporting people are maybe experiencing first episode of psychosis. The person will work in collaboration with the multi-disciplinary team the post-holder will act as Case Manager and carry continuing responsibility for: - the assessment of care needs and the development, implementation and evaluation of recovery-based care programs for young people and their families who may be experiencing psychosis.

Main duties of the job

1. CLINICAL and PROFESSIONAL I

The post-holder will have a high clinical profile that includes the provision of individual treatment, as well as having a global overview of interventions and clinical activities within the team.

The post-holder will undertake comprehensive initial assessment of the difficulties and needs of clients and their families, including mental state, symptom profile, social and vocational / educational functioning, quality of life and risk. They will be expected to use a range of assessment protocols, including formal measures and individualised interviews.

The post-holder will formulate a course of action based on initial assessment and implement specialised care plans for bio-psycho-social treatment and / or management of a client's mental health problems, based upon an appropriate conceptual framework of the client's problems.

They will ensure that all service users have an up-to-date Care Plan that has been negotiated with the service user to meet their individual needs.

As part of the MDT, the post-holder will implement a range of psychological, social and occupational interventions for individuals, carers, families and groups.

They will evaluate and make decisions about treatment, taking into account both theoretical and therapeutic models. Accountable for their own practice the post holder will decide when to report to their team manager or when to collaborate with a Specialist Worker or Service Clinical Lead.

CLINICAL GOVERNANCE / QUALITY ASSURANCE

Implements policies and proposes changes to working practices within the service, with an adherence to clinical standards of the service and emerging evidence in the treatment of early onset psychosis.

Support the development of the PALS Service and Service User Monitoring.

Report incidents and near-misses as required, promoting a learning the lessons culture.

The post holder will comply to the Trust standards for record keeping and input their own clinical information on standardised paper based and electronic record systems.

The post holder will be expected to assist the team manager with budgetary fidelity and document the use of agreed petty cash that may be handled by the post holder for use in patient activity.

The post holder may occasionally handle clients cash / valuables in the course of their work and care should be taken that this appropriately documented for audit purposes if required by the Trust.

EDUCATION AND DEVELOPMENT

Demonstrate the ability to monitor personal practice, ensuring live nursing registration is maintained including CPD & PREP requirements.

Ensure personal professional development promotes good practice, prevents poor practice, and intervenes in unacceptable practice.

Contribute to the development of a learning environment/ culture.

Attend relevant statutory training as identified by the Trust.

Actively participate in clinical practice development and may occasionally include the supervision of a student or attending Best Practice Groups and disseminating information back to the team.

Contribute to the development of an annual skills profiling of the team.

As co-ordinated by the Team Manager, the post-holder will occasionally deliver raising-awareness- teaching-sessions about the EI Service to assist in the Early Detection Strategy. This will be delivered as required to internal stakeholders and to partnership organisations in both the statutory and voluntary sectors, and self-help groups.

They will be expected to pursue their own professional training and education with particular emphasis on working with individuals, families and groups in Early Psychosis.

The post-holder will be expected to contribute to the research by providing caseload targeted data to the team manager and clinical lead.

The post holder will ensure they and those responsible/accountable to them, function within the parameters of their professional role.
