We have an exciting opportunity for a Band 3 Community Health Care Support Worker to join our Community Learning Disability Nursing Team.
The Team strives to provide forward thinking, high quality services that work in partnership with other agencies.The post holders will have experience of working with people with a learning disability and have enthusiasm to provide holistic person centred care. You will be working as part of a team where you will need to be able to communicate effectively with other staff, patients and their families/carers.
The job covers multiple bases across Derby City and Derbyshire. The post holders will need to meet the travel requirements of the post to complete patient visits in the community. A minimum of NVQ 3 or equivalent is essential. Those working towards this qualification will also be considered.
The successful candidates can expect to work within a friendly, professional and committed team, receive regular supervision and have training and development opportunities.
Main duties of the job
The Community Health Care Support Worker will support the Registered Nurses in all areas of the service's referral criteria. This covers a broad range including, Mental Health, Trauma and Emotional Distress, Complex Physical Health, Sexual Health and Relationships, Behaviour Support and Dementia.
Key aspects of the role include:
Working as part of a multi-disciplinary team to respond to nursing referrals and deliver person-centred care.
Working under the direction of the Lead or Registered Nurse to implement individual care plans, carrying out assigned tasks and interventions as required.
Reporting back any findings, changes, or concerns regarding service users' health, behaviour, or environment promptly to the Registered Nurse.
Contributing to risk assessments, treatment plans, and team reports.
Promoting safety, dignity, and wellbeing in line with Health & Safety policies.
Managing challenging behaviour using approved methods and care plans.
Be able to travel to visit people in their homes and within the community, including areas not accessible by public transport.
Maintaining effective communication (verbal, non-verbal, and written) with both service users and other professionals.
Liaising with professionals, carers, and agencies to ensure consistent and coordinated care.
Sharing information and model good practice to carers and colleagues.

Contribute as part of the multi-disciplinary team towards the implementation of care plans. Undertake a person-centred approach to meeting the health needs of people with a learning disability. To work as part of the multi-disciplinary team but will visit as a lone worker as appropriate, feeding back to the registered professional. To be aware of the risks, risk assessment and feedback any changes to the registered professional. Support registered professionals and the multi-disciplinary team to collate information to inform their assessment. To be responsible for their designated work on, a daily basis as agreed with the registered professional. To overcome barriers to communication using a range of communication methods, e.g., Makaton. To adapt information into a format that is accessible for people with a learning disability. Awareness of how to diffuse situations of aggressive and abusive behaviour guided by care plans, risk assessments, good practice and established guidelines, and report to the registered professional. To support access to health appointments following the nurse care plan in relation to desensitisation, reasonable adjustments and anxiety management. To encourage interest and engagement in a variety of daily activities, including self-care, domestic skills, productivity, and leisure Establish and maintain good relationships with service users, carers both paid and unpaid and staff across multi-agency boundaries. Have an awareness of, and be able to, recognise safeguarding concerns and escalate these to a registered professional. To have knowledge of Positive Behaviour Support approaches The post holder may be asked to undertake duties not directly highlighted within this job description. This will only be done when the skills and experience of the post holder meets the requirements of the role.
Leadership To work without direct supervision but under the plan of care of the registered professional. To work collaboratively with all relevant parties to promote effective team working. To act as a role model and participate in supporting new or less experienced staff and student nurses. To know when to escalate issues or concerns to the appropriate person.
