Job summary
We are seeking an experienced and motivated Learning Disability Community Nurse to play a key role in the ongoing development of our specialist community service. This is a community-based role, and you will be required to travel to visit service users on your defined caseload. You must be able to travel as required to meet the needs of the service, and please note that not all office locations or client care bases are accessible by public transport.
As part of the role, you will assess, plan, implement and evaluate person-centred care for individuals with learning disabilities, often with complex health needs, mental health difficulties, communication challenges and behaviours that can be difficult to manage.
Working collaboratively with service users, families, colleagues, and multi-agency partners, you will provide expert nursing input aimed at supporting people to remain in their community setting, maximising their health and wellbeing. You will demonstrate strong leadership, analytical and supervisory skills to ensure safe, effective and responsive care, while actively contributing to service improvements and innovations.
This role requires professionalism, accountability and a commitment to the NMC code, alongside enthusiasm for developing your expertise and supporting positive change within the service.
Main duties of the job

You will manage your own caseload, carrying out specialist assessments and creating person-centred care plans.

You'll travel within the community to visit people on your caseload, supporting them in their homes and local settings.

You'll support people with learning disabilities who may have complex health, communication and behavioural needs, helping them to live well in their communities.

You'll provide advice, education and advocacy to patients, families, carers and professionals.

You'll work closely with multi-agency partners to deliver safe, effective and joined-up care.

You'll recognise and act on safeguarding concerns, applying relevant legislation in practice.

You'll play a role in triage, referral and discharge processes to ensure the right support is in place.

You'll contribute to service development, audits and quality improvement projects.

You'll supervise and mentor colleagues and students, acting as a positive role model.

You'll maintain accurate records and support safe risk management in line with governance standards.

You'll take part in ongoing training and professional development, upholding the NMC Code.
