Job summary
This exciting opportunity has arisen to work specifically into the Older Adult inpatient wards supporting reduced length of stay and and improving the patient journey through our services.
As a registered practitioner you are personally accountable for your professional practice and have a legal/professional responsibility to service users, colleagues, employers and self.
We are looking for someone who can work autonomously and is able to liaise with external partners to improve patient outcomes.
It is Trust policy that for posts that are for a time limited period, applicants who are currently employed by an NHS organisation will only be appointed on a secondment basis and should have approval from their line manager this is possible before applying. Applicants who are not employed by an NHS organisation, will receive a fixed term contract.
Main duties of the job
You will work closely with the Matron and band 4 flow coordinators to ensure safe and timely discharge from Older Adult inpatient wards.
You will provide professional oversight to the band 4 flow coordinators based within in-patient settings.
You will represent the Older Adult wards at flow pathway meetings and liaise with system partners to achieve timely discharge from all 4 Older Adult inpatient wards.
You will demonstrate a professional image of nursing to promote a positive image of the Trust for staff, people under our care, carers and the community as a whole.
Within your role you will support the role of the Matron by:
o Providing strong clinical leadership, ensuring evidence based practice is delivered to improve the quality of and shape responsive discharge processes.
o Monitoring and ensure quality of Nurse placement assessments and provide education as required.
o Supporting junior colleagues within the process to discharge.
o Supervising, and promoting staff development for the band 4 flow coordinators.
You will have a broad understanding of the Derbyshire pathway discharge process and will form links into the wider system partner pathway.
The post holder will utilise support from Matron, HoN, Clinical leads and ward managers and report to Matron.
