Job summary
The Children and Families Act (2014) placed statutory responsibilities to improve the outcomes for children and young people with Special Educational Needs and Disability (SEND) from the ages of 0-25 on all services including health.
The SEND Clinical Co-Ordinator works closely with the Senior Management of the trust and the Integrated Health Board's (ICB) Designated Clinical Officer for SEND (DCO(SEND)) to ensure that the wide SEND agenda is addressed. However, as the SEND reforms have become more embedded there is recognition for the need for more resources into this area. Therefore, the role of the SEND Process Co-ordinator has been developed (this post)
The SEND Process Coordinator along with the SEND Clinical Co-ordinator:
- Provides a point of contact, for local authorities, schools and colleges seeking health advice on children and young people (C&YP) who have been identified as having a disability and when specialist health advice is required.
- Has oversight to ensure that requests for assessment, planning and health support are processed, recognizing the requirement to incorporate the transition to adult services and up to the age of 25.
- Provides assurance to the Trust that the performance targets are being met and of communication pathways and onward progression of work
Main duties of the job
Provides feedback to the Trust in relation to compliance against the SEND reforms for C&YP with SEND.
Oversee the Education, Health and Care Plan (EHCP) referral process
Liaise closely with the SEND Clinical Co-ordinator to ensure that practices are in line with SEND Reform requirements.
Be an active member of the Trust SEND stakeholder group.
Act as a point of contact for enquiries relating to EHCP process where clinical or data-related advice is required.
Have an overview of requests for health input to EHCPs to ensure that all health professionals who are involved with CYP are identified.
Have oversight and assist in Trust planning for the health aspects of the SEND local area inspections.
Liaise with other administrators, managers and clinicians where relevant, in relation to SEND.
Act as a point of contact/coordinator in relation to transition C&YP and Adult services in support of the delivery of the SEND requirements.
Have oversight of the pathways and processes of EHCP, ensuring best practice and throughput
Contribute to ensuring Quality Assurance processes are in place, with regular performance reports
Ensure effective working relationships are in place with health providers in order to support C&YP with SEND across the 0-25 age range.

You will be part of the Children's Complex Health and Therapist teams based at St Paul's House and have the support of the SEND Clinical Co-Ordinator. This post can be flexible to fit in with other roles you may have in the Trust.
