Then come to work for Derbyshire Healthcare NHS Foundation Trust as a Band 5 Registered Nurse, within our 0-5 Children's Team supporting and working with families and children who have been diagnosed with a disability.
As a Band 5 Registered Nurse you will support the specialist Health Visitors, working with families and children, contributing and completing relevant assessments, supporting the family with packages of care, for example, sleep and behaviour within a multi-disciplinary team setting to meet the prioritised needs of the families and children, which will include delivering groups to meet the needs identified.

Key result areas
- To participate in the assessment, planning, delivery, and evaluation of the holistic health needs of children with complex disabilities identifying levels of care using universal partnership plus tier. E.g., Completing sleep assessments, toileting assessments, early help assessments and team around the family meetings.
- Support with and contribute to the delivery of Core Services for children with complex disabilities in Derby city area e.g., management of complex sleep, behaviour, feeding difficulties and toilet training.
- Deliver the service sensitively and flexibly in response to the needs of the family and the organisation. This is achieved by offering contact by telephone, home visits, groups and across settings such as clinic, childrens centres, community rooms etc.
- To hold child protection responsibilities within a caseload with support/supervision from the named Disability Health Visitor in line with service delivery and local policies.
- To support the service with improving outcomes for children with complex disabilities and assist in developing new innovative ways of delivering information/training to parents such as in groups settings e.g., Sleep, behaviour, feeding difficulties, toilet training etc.
- To communicate in a skilled and sensitive manner, complex information regarding assessment and analysis of health needs of children with complex disabilities and their families in routine practice and highly emotive situation.
- Work in partnership with key agencies to meet the health and social care needs of children with complex disabilities and families, working within safeguarding local policies.
- To support, mentor and supervise appropriate members of the team to ensure PADRs and supervisions are completed in a timely manner.
- Ensure training passport is kept up to date and ensure professional development and practice is up to date and safe.
- To maintain contemporaneous records using DHcFTs IT record systems and write reports as required.
- To lead by example with regards to ones own conduct, professionalism, and clinical practice be a good role model.
To adhere to NMC guidelines for professional practice, DCSB policies, and DHcFT policies. To work in ways which are sensitive and appropriate to the need of children with complex disabilities and their families / carers with a wide range of ethnic, cultural, and religious backgrounds. Contribute towards the delivery of performance targets set by DHcFT and the local commissioners as identified in the performance management framework. To write reports, collect data and provide statistical returns as requested to assist DHcFT with monitoring and performance management of the 0-5 Senior Disability service to maintain quality standards and audit.
