Job summary
This is a full time post, but part time hours would be considered.
The post covers the early discharge function of CRHT in both the south county and city crisis and home treatment teams.
We have an exciting opportunity for a Mental Health Nurse, Occupational Therapist or Social Worker with recent and relevant, post-registration experience.
This well-established, multi-disciplinary team which offers a high quality assessment and home treatment service as an alternative to hospital admission, to individuals experiencing acute mental health crisis. This role is specific to mainly work within the early discharge part of the service, specifically focusing on the discharge of patients from hospital.
Main duties of the job
- To provide first line assessments of people experiencing acute mental health difficulties the team acts as gatekeepers to acute admissions.
- To be able to rapidly assess people with mental health problems whom are vulnerable to hospital admission
- To provide specialist crisis assessments looking at the focus being on "hospital at home" with a focus on a safe discharge from a hospital setting.
- To co-ordinate workload on a shift by shift basis and plan the shifts activities and team accordingly.
- To participate in team and ward MDMs and liaise with other services / agencies.

- This is a 24 hour service across a 7 day week, 365 days per year and as such the successful candidates will be expected to work as part of a 24 hour rota.
- As a large part of the team's work involves assessing people in mental health crisis within a variety of settings and challenging situations, applicants must be familiar with and engage in assessments on a regular basis and be able to meet the travel requirements of the post.
