Job summary
Kedleston unit is a 20 bedded male, low secure unit, situated on the Kingsway site in Derby.
We are excited to be able to advertise this great opportunity for a Social Worker to add to our multi-disciplinary team.
The Kedleston unit provides a 24-hour inpatient service and we help adults with severe and enduring mental illness, who have a forensic or significant risk history and require a longer period of assessment and stabilisation whilst we support them to transition back into the community.
The successful applicant will be expected to support the multi-disciplinary team in providing social care input, providing social circumstances reports, engaging with the patient, team and their families and assessing their patients social care needs alongside to the wider, multi-disciplinary team to aid transition into the community.
The aim being to provide an effective, high quality and evidenced based rehabilitation and recovery service, which is unpinned by the promotion of a hopeful, compassionate and expressive environment, offering choice and opportunities, and prioritising service users goals whenever possible.

Main duties of the job
The post holder will:
- Work with the team, driving change and offering assurance that our care meets the standards not only expected of us, but that our patients, relatives and carers deserve.
- Be responsible for establishing social care needs for all of our patients, across two wards - Curzon, an admission and assessment ward and a rehabilitation ward, Scarsdale.
- You will have the opportunity to develop the role to work best to meet the social care needs of the unit, and will be responsible for ensuring that local and national standards and targets are met.
- Be responsible for ensuring that social circumstances reports are completed prior to all required hearings.
- Work effectively within a multi-disciplinary setting and will ensure that effective co-ordination exists and is maintained, between the various stakeholders involved.
- You will be actively involved in working with families and carers and their input into their relatives lives.
- You will be a key part of helping the discharge process for our patients, assessing the social care needs as part of rehabilitation and as part of discharge planning.
- You will be part of risk assessments and team meetings. There is also opportunity to be involved in quality improvement projects within the unit.
