Derbyshire Healthcare have an exciting opportunity for an Area Service Manager to work in Transforming Care and Neurodevelopmental Services.
The post holder will be an Area Service Manager who is employed by DHcFT but is a system-wide post reporting out on patient details, working with case managers, ICB Quality Directorate, NHSE (both regional and National NHSE teams).
The post holder will be required to manage the patients on the Transforming Care cohort including those in out of area beds, locked rehabs and Assessment and Treatment Units.
The post holder will be responsible for delivering effective Dynamic Support Register function on behalf of Derbyshire and the escalations to CTR/LAEPs.
Main duties of the job
TCP manage the reporting out of information, including personal and sensitive patient information from across the JUCD.
Understands the use of MH, LD and children's services, both internal and external to Derbyshire to support the ICB's assurance of appropriately commissioned services for individuals (placements, packages of care, etc.
Attend and support all relevant system workstreams, aligning governance and assurance process to Derbyshire wide strategy.
