Job summary
The Trust's rehabilitation service is evolving, and we have a wonderful opportunity for a Lead Nurse in the service.
The team provides 24-hour inpatient support and help to 23 adults with severe and enduring mental illness who have traditionally found it difficult to spend long periods of time out of other inpatient settings.
The aim being to provide an effective, high quality and evidenced based rehabilitation and recovery service, which is underpinned by the promotion of a hopeful, compassionate and expressive environment, offering choice and opportunities, and prioritising service users goals whenever possible
Main duties of the job
You will provide a professional role model for the delivery of high-quality nursing care.
You will ensure that service users have an up to date evidenced-based care plan that meets the needs of service users
You will be responsible for reviewing of care plans and the completing and reviewing risk assessments. Be responsible for ensuring the Care Programme Approach is applied topractice and that opportunity is fully provided to support service user/carer involvement in decision making processes.
