Job summary
As a partner of Derbyshire's Integrated Care Board, it is our vision to create a combined ND assessment pathway that works across all ICB NHS Trusts to ensure that children and young people see the right person at the right level, with the right skills at the right time.
In order to do this, we will be working across organisational boundaries, to the same standards and putting the patient at the heart of all that we do.
As an ICB we have agreed to work together with the aim of developing a Derbyshire-wide Neurodiversity offer, where children, young people and adults can expect improved access to advice, support and specialist assessment services regardless of their post code.
Please note that whilst this post may be hosted by the employing organisation, we will be working towards a Provider Partnership that enables these posts to span across the ND assessment services (including CAMHS, Psychology, Community Paediatricians, Specialist ADHD Nurse Teams and Speech and Language Therapies), embracing the concept of a multi-disciplinary team as recommended by NICE.
Main duties of the job
The post holder will be required to link closely with an existing triage Nurse & the community hubs, working within a multi-skilled team of professionals supporting the triage of City and County referrals for assessment.
They will be responsible for clinically triaging referrals for children and young people aged 5-16 years, who present with possible emerging Neuro-developmental difficulties that may be indicative of a Attention Deficit Hyperactivity Disorder and Autism Spectrum Disorder and require an assessment.
The post holder will be required to analyse and understand a range of supplementary documents that accompany the referral, have extensive knowledge of the criteria for assessment.
They will be required to filter referrals that may not require the need to process through a diagnostic pathway and signpost, for appropriate support for the children and their families so they receive the right care at the right time.
The post holder will be required to co-ordinate with a range of different agencies and professionals, communicating the care and support required with the relevant stakeholders.
Where referrals require the need to follow a diagnostic pathway, it will be required to ensure all the required supplementary assessment tools are present and signpost support for the children and their families whilst they are waiting for the assessment to commence.
