The position will consist of a smal caseload. The caseload itself will have complex people for you to support through their journey to recovery.
An exciting opportunity has arisen to recruit an experienced, Mental Health Nurse, to work in our friendly Derbyshire Dales Living Well team.
Please note: this role will cover the Derbyshire Dales and will work out of 2 bases, St Oswald's Hospital in Ashbourne and The Ritz Building in Matlock.
Please do ring if you have any questions and/or queries.The post involves the provision of care to service users in the Assertive Outreach element of the Team.
This includes using a range of appropriate interventions to work with service users who have long term mental health problems are difficult to engage with services and treatment.
The post primarily involves undertaking the role of Care Co-ordinator for people who are experiencing severe and enduring mental ill-health, alongside significant difficulties with engagement and/or a forensic history.
The client group may also have drug and alcohol problems.
Specialist forensic supervision is provided for this group of service users, along with Assertive Outreach peer supervision and usual supervision structures.
Main duties of the job
You will need a thorough knowledge of the Care Programme Approach and be able to demonstrate expertise in clinical risk assessment and management working in a community setting.
Experience of working with individuals with a forensic history and substance misuse issues would be an advantage.
We are looking for experience and commitment together with a desire to make our service the best we can.
It is essential that you have excellent communication skills and that you operate as a well-integrated member of the multi-disciplinary team maintaining a person-centred focus.You will need to have the ability to transport yourself and service users across a wide geographical area without compromising service needs.
