North East Derbyshire PCN is looking to recruit a Mental Health Practitioner to its expanding team.
Our aim is to change the face of mental health provision within Primary Care and replace the traditional GP model with a more diverse in-house Mental Health Team.

This role is an exciting opportunity to work closely with GP surgeries and you will be able to help, educate and support teams within primary care and there will be opportunities of innovation as well as continuing training and development opportunities.
Main duties of the job
- To provide a safe and effective service open to referrals from clinicians across the PCN.
- To undertake individual assessments of a client's needs and form plans for ongoing care and management.
- Have some level of knowledge of talking therapies such as CBT, DBT and mindfulness.
- Where appropriate to refer to and involve other services to provide the most appropriate care.
- To maintain a case load of patients. This will be a mixed caseload: some people with enduring needs may be under review for long periods, others however, may only be on the case load for the duration of a short-term crisis.
- To have the ability to help service uses overcome social inequality related to mental health.
- Offer clinical support to non-mental health specialists within the PCN.
- To use and create accurate clinical records and include clear plans of actions and assessments of risk
- To be conscious of one's own clinical expertise and knowledge base, to work within this and where appropriate identify learning needs and opportunities
- Skilled in decision making and time management.

'Coming from crisis MH services to primary care was a big change and a daunting thought initially. NEDPCN have been supportive and welcoming .The service is helping bridge that gap between services. I have had frequent support from all agencies including Derbyshire healthcare, NEDPCN and the GP Practices in starting up the service and supporting my own professional development.
