Older Adult Crisis Practitioner

Job summary

The Older Adult In-reach and Home Treatment Team is seeking an enthusiastic and compassionate individual to join us in Chesterfield.

This service works with Older Adults (age 65+) who are experiencing a crisis in relation to a functional mental health condition.

The primary goal of the IRHTT is to support service users within the community (both in their own homes and in 24-hour care), to prevent hospital admission where possible.

In addition, the IRHTT regularly attends in-patient wards to work with service users who are close to discharge, to reduce their length of stay in hospital.

Main duties of the job

• You must have a full UK Driving License and the means to travel around the community.
• You will be able to make complex decisions, lead a shift, cover duty, formulate detailed assessments and person-centred care plans to assist in the prevention of hospital admissions.
• Comprehensive risk assessing and care planning.
• Experience of working with functional mental health illness is essential.
• The successful applicant will work within a supportive MDT.
• Professional development, learning, supervision, wider support and training is provided.
• The service operates 7 days per week, 365 days a year. The working hours are 9am-5pm.
