Main duties of the job

  • Provide a caring and compassionate service with clear boundaries and expectations
  • Complete initial conversations to help people plan what they need
  • Hold a small case load of people with deteriorating/fluctuating mental health problems and works alongside them to help achieve their goals
  • Discuss initial enquires with anyone in the system
  • "Does" safe-guarding - it's everybody's business.
  • Consider capacity and choice in every contact
  • Attend Living Well Team meeting structure - Relative to WTE
  • Bring new ideas and innovations to the table

Please note: this role will cover the Chesterfield area and although the main base will be at Bayheath House Chesterfield Derbyshire, you will be required to work flexibly across the entire area and will have the opportunity to work from different Trust bases.

You will need to have the ability to transport yourself and service users across a wide geographical area without compromising service needs.
