Main duties of the job

You will be a member of a collaborative, well-established, progressively developing Service within Derbyshire, providing a 24/7 crisis and resolution home treatment for all adults, aged between 18 and 65, who require a clinical consultation for a mental health crisis.

The team consists of a variety of workers from multi-disciplinary backgrounds including nurses, medics, occupational therapists, social workers, research assistant, pharmacy and psychology who work alongside each other to ensure excellent care is given to all who come into contact with the service.

You will promote a dynamic and innovative way of working, providing an expert led triage function based on a collaborative referral process inclusive of a prompt and timely response and leads onto assessment and home treatment functions as and when clinically necessary and/or discussion around most appropriate needs for a person.

As a Band 5 Registered Practitioner, the post holder will be consolidating pre-registration education and developing enhanced knowledge and skills within the patient pathway, and contributing to evidence-based professional practice.

The post holder will be responsible for establishing operational and clinical service mechanisms that support safe, responsive and high quality care throughout the twenty-four hour period.

The post holder will present a positive, professional image to service users, carers and the community as a whole.

You will have responsibility of supervision for assistant practitioners relevant to your profession and sphere of knowledge.

As a member of the Crisis Resolution, Home Treatment Team you will be expected to adopt the philosophy of the service and provide flexible, responsive, proactive and interactive assessment, care and management to individuals experiencing early signs of relapse or new acute phase mental health problems. All interventions will be short term; solution- focused and build on maintaining the safety, well being and empowerment of the service user and their Carers / family during this period.

The team will provide a proven research based alternative to hospital admission, and where admission is required will act as gatekeeper to the admission and will facilitate early discharge.

The post holder will be able to sustain a respectful, consistent and reliable therapeutic relationship with service users that enhance their ability to build social networks and work in partnership with Housing Association, Mental Health Teams, Support Networks, Primary Health Care Teams and other Statutory, Voluntary and Independent agencies, ensuring social inclusion for service users.

It is essential that you, as the post holder, will embed trust values in care delivery services, as this is necessary to achieve our ambition as being one of the top five providers of health care within the country.
