Job summary

The community support worker role is an integral part of the Reconnect service. It will enhance the work of the Reconnect Team in screening, liaising and supporting people whilst in custody in preparation for release into the community.

The support offered will aim to largely in the community in supporting service users to engage with services such as health, mental health, housing, etc, post-release. It will include support to attend appointments with agencies referred to, link into physical health services, etc, supporting an effective transition into the community.

All work will be carried out under the supervision of the Reconnect Practitioners.

The successful applicant will need to apply for Criminal JusticeSector/Prison vetting and Derbyshire Police vetting clearance and haveit approved before a contract can be agreed

Main duties of the job

The Reconnect Service aims to increase access to and uptake of healthcare or relevant support services for individuals who have health vulnerabilities upon release from prison or immigration removal centres, who would otherwise struggle to engage.

The post is situated within the Forensic Division, as part of the Offender Health Directorate.

The post holder will be located in one of the community team bases and will largely be based in the community. However, there will be some element of the role requiring attendance at prison establishments (locally and occasionally nationally).

The post holder will work with people who a range of vulnerabilities, such as offending, homelessness, substance misuse, physical health needs, etc.

The post holder will work alongside the practitioners assisting in the screening, liaising and supporting service users with needs.

The main focus of the role is to support those transitioning into the community following a period in custody, this will include assertive follow up.
